Pübertede gonadal steroid ve lipoproteinlerin değişim ve etkileşimleri

Amaç: Püberte, insan yaşamında dramatik değişikliklerin oluştuğu dönemdir. Bu çalışmada, pübertal gelişim sırasında, gonadal steroidler, lipoprotein düzeyleri ve antropometrik ölçümlerdeki değişim ve etkileşim belirlenmeye çalışıldı. Gereç ve Yöntem: Bu kesitsel bir çalışmadır. Çalışmaya 10-15 yaş arası elli dokuz kız ve altmış dokuz erkek okul çocukları dahil edildi. Çocukların püberte evreleri Tanner evrelerine göre değerlendirildi. Boy ve kiloları ölçüldü. Vücut kitle indeksi (VKİ) hesaplandı. Gonadal steroidler (östradiol, serbest ve total testosteron), lipid ve lipoprotein düzeyleri belirlendi. Bulgular: Kızlarda erken pubertede total kolesterol ve HDL-Kolesterol (HDL-K) düzeyleri anlamlı artış gösterdi (p<0.05). Apolipoprotein A-I (ApoA-I), Apolipoprotein B (ApoB), Lipoprotein a (Lip a) ve LDL-Kolesterol (LDL-K) düzeylerinde püberte boyunca anlamlı değişiklik olmadı (p>0.05). Erkeklerde erken pübertede LDL-K’de anlamlı artış saptandı (p<0.05). Püberte sonunda tüm lipoprotein düzeyleri anlamlı arttı (p<0.05). Püberte öncesinde kızlarda LDL-K erkeklere göre anlamlı yüksekti. Püberte sonunda erkeklerde LDL-K, Lip a ve trigliserid düzeyleri kızlara göre anlamlı yüksek bulundu (p<0.05). Kızlarda östradiol düzeyleri ile tüm lipid düzeyleri arasında anlamlı ilişki olmasına karşın, en güçlü ilişki HDL-K ile saptandı (r=0.58). Erkeklerde de östradiol ile tüm lipid parametreleri arasında ilişki vardı. Testosteron ve aterosklerotik lipidler arasında anlamlı ilişki vardı (p<0.05). VKİ ile LDL-K arasında orta dereceli anlamlı ilişki belirlendi (r=0.28, p<0,05). Erkeklerde VKİ ile trigliserid, LDL-K ve Apo-B arasında orta dereceli anlamlı ilişki belirlendi (r=0.34, r=0.31, r=0.29, p<0.05). Sonuç: Pübertal gelişim sırasında kızlarda antiaterosklerotik, erkeklerde aterosklerotik lipid düzeyleri artar. Östradiol kızlarda antiaterosklerotik lipidleri arttırırken, erkeklerde tüm lipidlere etki eder. Testosteron erkeklerde aterosklerotik lipidleri arttırır. VKİ artışı her iki cinste de aterosklerotik lipidleri ve özellikle LDL-K’yi arttırır. Purpose: Puberty is a period in which dramatic changes occur in human life. We aimed to determine the changes and interaction of gonadal steroids, lipoprotein levels and anthropometric indices during pubertal maturation. Materials and Methods: This is a cross sectional study. Fifty-nine girls and 69 boys among school children aged between 10 and 15 years were included in this study. Pubertal development of children was evaluated according to Tanner pubertal stages. Their weights and heights were measured. Body mass index (BMI) was calculated. The gonadal steroids (Estradiol, free and total testosterone), lipid and lipoprotein levels were evaluated. Results: Total and HDL cholesterol (HDL-C) levels were significantly increased at early puberty in girls (p<0.05). Apolipoprotein A-I (ApoA-I), Apolipoprotein B (ApoB), Lip a and LDL-cholesterol (LDL-C) levels did not significantly change during puberty in girls (p>0.05). LDL-C levels were significantly increased at early puberty in boys (p<0.05). All lipoprotein levels increased at the end of puberty (p<0.05). The LDL-C level of girls was higher than boys at prepubertal stage. The levels of LDL-C, Lip a and trigliserid were higher in boys than in girls at the end of puberty (p<0.05). Although estradiol levels correlated with all lipid levels in girls, the most strongest association was between estradiol and HDL-C (r=0.58). Estradiol correlated with all lipid parameters in boys also. There was a significant correlation between testosteron and atherosclerotic lipid parameters (p<0.05). A moderate correlation between BMI and LDL-C was detected in girls (r=0.28, p<0.05). In boys, there was moderate correlation between BMI and trigliserid, LDL-C and Apo-B levels (r=0.34, r=0.31, r=0.29). Conclusion: During pubertal maturation, antiaherosclerotic lipid levels increase in girls, whereas atherosclerotic lipid levels are increased in boys. Estradiol affects all lipid levels in boys, while it increases antiatherosclerotic lipids in girls. Testosteron increases atherosclerotic lipids in boys. Increase of BMI increases atherosclerotic lipids, particularly LDL-C in both sexes.
