Talusun osteokondral lezyonlarında artroskopik drilleme yönteminin etkinliği
Abstract
Amaç: Talusun osteokondral lezyonlarındaıartroskopik drilleme yönteminin sonuçlarını değerlendirmek. Hastalar ve Yöntem: Talusta osteokondral lezyon nedeniyle opere ettiğimiz ortalama yaşı 27 (17-40) olan, 6’sı erkek, 5’i kadın 11 hastanın 11 ayak bileği çalışma grubunu oluşturdu. Hastalar ortama 39 ay (10-96) izlendi. Talustaki osteokondral lezyonların 6’sı anteromedial bölgede, 3’ü anterolateral bölgede ve 2’si posteromedial bölgede görüldü. Radyolojik incelemelerin ışığında, Berndt ve Harty sınıflamasına göre osteokondral lezyonların yedisi evre II, ikisi evre III, ikisi evre IV olduğu tespit edildi. Ayak bileği artroskopisinde Outerbridge sınıflamasına göre 5 hastada lezyonun evre II, 4 hastada evre III, 2 hastada evre IV olduğu görüldü. Evre II lezyonlara artroskopik drilleme, Evre III ve IV lezyonlara ise fragmanın debridmanından sonra drilleme uygulandı. Hastaların operasyondan elde ettiği kazanım Amerikan Ortopedik Ayak-Ayak bileği Derneği (AOFAS) skoru ile değerlendirildi. Bulgular: Operasyon öncesi ortalama 66 puan (46-74) olan skor değerleri, izlem sonunda ortalama 87 puana (78-97) yükseldi (p<0.05). Radyolojik olarak 11 ayak bileğinin 4’ünde lezyonun gerilediğini gördük. Kalan 6 hastada radyolojik olarak anlamlı değişiklikler saptanmadı. Kalan bir olguda (evre IV lezyon) ayak bileğinde eklem yüzlerinde dejenerasyon tespit edildi. Tartışma: Talusun osteokondral lezyonlarının tedavisinde artroskopik drilleme tekniği etkili ve yararlı bir tedavi metodudur. Ancak, klinik rahatlamaya radyolojik düzelme her zaman eşlik etmez. Objective: To evaluate the outcome of arthroscopic drilling for the treatment of osteochondral lesion of talus. Patients and Method: Eleven (six males and five females) patients with a symptomatic osteochondral lesion of the talus were operated with arthroscopic drilling. They were followed postoperatively for ten to ninety-six months (mean, 39 months) The mean age of the patients was 27 years (range, 17-40 years). There were six lesions on the anteriomedial aspect of the talus, the three anteriolaterally and two posteriomedially. According to the radiographic classification system of Berndt and Harty, seven ankles had Stage II lesions, two had Stage III and two had Stage IV. The lesions were also classified according to the their arthroscopic appearance by using Outerbridge classification. Arthroscopic debridement and drilling were performed in Stage III and IV lesions. Stage II lesions were treated with arthroscopic drilling only. The outcome was evaluated according to the American Orthopaedic Foot and Ankle Society (AOFAS) score. Results: The average preoperative AOFAS score of 66 points (46 to 74) improved to 87 points (78 to 97) at the last follow-up. The radiographic findings showed that four lesions were improved, six lesions were unchanged and the only one stage IV lesion was deteriorated. Discussion: Arthroscopic drilling for the treatment of the osteochondral lesions of the talus is an effective and useful method. However, the improvement of clinical outcome is not always accompanied with radiological improvement.
