Patellofemoral eklem patolojisi ile alt ekstremite torsiyonel deformitelerinin ilişkisi
Abstract
Giriş: Patellofemoral hastalıkların nedenleri çok çeşitlidir. Femoral anteversiyon ve eksternal tibial torsiyondaki artışın diz ön ağnsına neden olabileceği yönünde görüşler vardır. Bu çalışmanın amacı, patellofemoral instabilite ve diz ön ağrısı bulunan olgularda alt ekstremite torsiyonel deformitelerini araştırmak ve bunu normal bireylerle karşılaştırmaktır. Hastalar ve Yöntem: Bu amaçla 27 bireyin 40 dizi kullanılmıştır. Olgular klinik tanılarına göre üç grupta toplanmıştır. Grup l patellar instabilite semptomları olmayan konservatif tedaviye yanıt vermeyen diz ön ağrılı 10 hastanın 12 dizinden oluşturuldu. Grup 2 klinik ve bilgisayarlı tomografi (BT) bulguları majör instabiliteyi gösteren 8 hastanın 12 dizini kapsamaktaydı. Grup 3 ise kontrol grubu olarak sağlıklı 9 bireyin 16 dizinden oluştu. Tüm gruplarda femoral anteversiyon ve tibial torsiyon BT yardımıyla ölçülerek alt ekstremitenin torsiyonel durumu deeğerlendirildi. Bulgular: Tibial torsiyon Grup l'de 31,83±6,93, Grup 2'de 38,42±3,78, Grup 3'de 39,3+8,09 derece olarak bulundu. Grup 2 ve Grup 3'le karşılaştırıldığında Grup l'de anlamlı olarak azalmış eksternal tibial torsiyon bulundu (pl<0,0028 ve p2<0,001). Femoral anteversiyon, Grup l'de 19,67±6,11, Grup 2'de 15±4,82, Grup 3'de 18,31 ±5,59 derece olarak tespit edildi. Femoral anteversiyon değerlerinde gruplar arası istatiksel anlamlı fark bulunmadı (p > 0,05). Tartışma: Alt ekstremite torsiyonel deformiteleri patellofemoral ağrı veya instabilitede etyolojik primer faktör olarak değerlendirilemez. Introduction: There are various causes of the patellofemoral disorders. The increased femoral anteversion and external tibial torsion are emphasized as probable causes of anterior knee pain. The aim of this study is to compare two group of patients having anterior knee pain and patellofemoral instability with healthy controls in terms of lower extremity torsional deformities. Patients and Method: For this purpose, we evaluated 40 knees of 27 cases. Cases were assigned to one of 3 groups according their clinical diagnosis. Group l consisted twelve knees of ten patients who have anterior knee pain not responding conservative treatment for a long period. Group 2 included twelve knees of eight patients with objective patellar instability proven with computed tomography (CT) and physical examination. Group 3 as control was composed of sixteen knees of 9 healthy volunteers. Femoral anteversion and tibial torsion were measured for lovver extremity torsional status by CT in ali 3 groups. Results: Mean tibial torsion was 31,83±6,92 in Group l, 38,42±3,78 in Group 2 and 39,3±8,09 degrees in Group 3. Group l demonstrated significantly lovver values of external tibial torsion compared to Group 2 and Group 3 (P<0.0028 and P<0.001 respectively). Mean femoral anteversion angle vvas 19,67±6,11 in Group l, 15±4,82 in Group 2 and 18,61 ±5,59 degrees in Group 3. There were no significant differences among the groups (P>0.05).Discussion: These findings suggest that the lower extremity torsional deformities cannot be regarded as primary etiological factor for anterior knee pain syndrome and patellofemoral instability
