Tuba uterina gamet taşınmasında, fertilizasyonda ve erken embriyo gelişiminde rol oynayan bir bölgedir. Tuba uterina epitelinin östrojen ve progesteron konsantrasyonlarındaki değişikliklere cevap olarak yapısal değişiklikler gösterdiği bilinmektedir. Bu çalışmada, menstrüel siklusun farklı dönemlerinde ve erken gebelik döneminde alınan insan tuba uterina istmus bölgesi epitel hücrelerinde oluşan değişikliklerin morfometrik ve ultrastrüktürel olarak incelenmesi amaçlanmıştır. Sağlıklı onbeş kadından tüp ligasyonu sırasında alınan doku örnekleri, rutin ışık ve elektron mikroskop takip yöntemleriyle hazırlandı. Vakalar son menstrual periyodlarının başlangıç tarihlerine göre erken-geç proliferasyon, erken-geç sekresyon dönemi ve erken gebelik olmak üzere 5 gruba ayrıldı. Işık mikroskobu ile, proliferasyon dönem boyunca tuba uterina epitel kalınlığında, silyalı hücrelerin sayısında ve boyutunda artış saptanırken, geç sekresyon ve gebelik dönemlerinde bu değişikliklerin gerilediği görüldü. Elektron mikroskobu ile incelemede, menstrual siklus boyunca salgı yapıcı hücrelerin silya içeren hücrelere göre daha fazla ultrastrüktürel değişiklikler gösterdiği izlendi. Sonuçlar, menstruel siklus ve erken gebelik boyunca tuba uterina epitel hücrelerinde hem hacim hem de hücresel değişiklikler olduğunu ortaya koymuştur. Bu değişikliklerin daha çok ovulasyonla birlikte görülmesi, tuba uterinanın fertilizasyon ve erken embriyo gelişimi sırasında önemli rol oynayabileceğini göstermektedir. The fallopian tube is the site of gamete transport, fertilization, and early embryo development. Epithelium of the fallopian tube is known significant morphological variations in response to changes in estrogen and progesterone concentrations. The aim of this study is to compare morphometric and ultrastructural changes in isthmic portion of human fallopian tube epithelium at different stages of the menstrual cycle and early pregnancy. Fifteen healthy women undergoing tubal ligation were included in this study. During the menstrual cycle the epithelial height and volume were observed on serial sections and were calculated by stereological technique. While the height of ciliated cells was increased during the proliferative phase, maximal ciliation was attained around the time of ovulation. Also, deciliation and the decrease of epithelial height were observed after ovulation and early pregnancy. By electron microscopic examination, secretory cells were seen to have more marked ultrastructural changes than ciliated cells during the cycle. The evidence suggests that the epithelium of the fallopian tube had morphological changes that consist of changes in both epithelial volume and cell population during the menstrual cycle and early pregnancy. Because these differences especially were observed with ovulation, the fallopian tube may play a crucial role during fertilization and early development of embryo.
