Amaç: Idiopatik trombositopenik purpura (ITP); trombositopeni, trombosit ömründe kısalma, plazmada anti-trombosit antikorların varlığı ve kemik iliğinde megakaryoşit sayısında artma ile karakterize otoimmun bir hastalıktır. ITP'nin tedavisi bireysel temellere ve trombositopeninin derecesine bağlıdır. Anti-D, büyük oranda eritrositlerin Rho (D) antijenine karşı gelişmiş antikorları içeren bir gama globulindir. Klinik çalışmalar, splenektomi yapılmamış, Rh (+) ITP'H hastalarda intravenöz anti-D'nin güvenli ve etkili bir tedavi olduğunu göstermektedir. Bu çalışmada akut ve kronik ITP'H çocuk hastalarda anti-D tedavisinin etkinliği ve güvenilirliğinin araştırılması amaçlanmıştır. Yöntem: İzmir Dr. Behçet Uz Çocuk Hastanesi Hematoloji bölümünde 4-78 aydır izlenen, 2-15 yaş arasındaki, 15 ITP'H hastaya 50 xg/kg dozunda anti-D (WinRhoSDFTM) intravenöz olarak uygulandı. Tedavi öncesi bazal, tedavi sonrası 1., 3. ve 7. gün trombosit sayıları ve hemoglobin değerleri tespit edildi. 7. gündeki trombosit sayısı >150xl09/L tam yanıtlı; 20-150xl09/L kısmi yanıtlı; <20xl09/L tedaviye yanıtsız kabul edildi. Tedavi sonrası 1-53 hafta izlendi. İstatistiksel analizde Fisher-exact test kullanıldı. , Bulgular: Dokuz hastada parsiyel, üç hastada tam yanıt alındı. Tüm olgularda 7. gün trombosit sayısı 79xlO9/L olup, median 20xl09/L'nin üzerinde seyretti. Klinik olarak akut veya kronik seyirli olgularda tedaviye yanıt yönünden istatistiksel fark bulunmadı. Tüm olgularda Hb değerlerinde ortalama 1.2 ± 1.0 gr/dl düşme oldu. Anaflaksi gibi ciddi bir yan etki gözlenmedi. Bir olgu eşlik eden gastrointestinal sistem kanaması nedeniyle transfüze edildi. Sonuç: Akut ve kronik ITP'H çocuklarda Anti-D, güvenli, kolay ulaşılabilir, ucuz ve etkili bir tedavi yöntemidir. Aim: Idiopathic thrombocytopenic purpura (ITP) is an autoimmune disease characterized by thrombocytopenia because of acceleration in platelet destruction, presence of antiplatelet antibody in the plasma, increased megakaryocytes in the bone marrow. ITP treatment depends on individual basis and degree of thrombocytopenia. Anti-D is a gamma globulin (Ig G) fraction containing a high proportion of antibodies against Rho (D) antigen of the red blood cells. Nowadays clinical studies have shown that intravenous anti-D is a safe and an effective treatment for Rh positive nonsplenectpmized (patients with ITP. The aim of this study was to evaluate the efficacy and reliability of intravenous anti-Rho (D) treatment in acute and chronic ITP of children. Methods: Fifteen pediatric patients with chronic and acute ITP aged between 2-15 years who were followed-up at Izmir Dr. Behçet Uz Children's Hospital Hematology Department were included in the study. Patients were treated by 50 pig/kg/dose anti-D (WinRhoSDFTM). The platelet counts and hemoglobin levels were detected before and afier treatment on the first, 3rd and 7th days. Platelet counts > 15OxlO9/L on the 7th day "complete response" 20-150xl09/L "partial response" and platelet counts <20xl09/L were accepted as "no pesponse". Patients were followed for 1-53 weeks after therapy. For statistical analysis Fisher-exact test was used. Results: Partial and complete responses were observed in 9 and 3 patients respectively. Median platelet count of all the patients on the 7th day was 79xlO9/L and the platelet counts of the patients continued to be over 20xl09/L for median 3 weeks (&52 weeks). No statistical difference was found between the chronic and acute ITP patients in terms of response to anti-D therapy. Mean Hb deceleration of 1.2 ± 1.0 gr/dl was observed after therapy within the whole patients and only one patient had to be transfused because of the gastrointestinal bleeding. No serious side effects were seen. Conclusion: Anti-D is a safe, convenient, inexpensive and effective therapy for childhood ITP.
